HELPFUL FORMS:
We frequently get requests for some of the more commonly used forms from the IRS, our tax organizer and the forms checklist.. We are happy to provide copies of them here for the convenience of our clients.

Bank & Identity Authentication Form
This form is in two parts.

Fill out the top Direct Deposit/Electronic Funds Withdrawal Information ONLY if you would like to have your refund direct deposited into your checking account.

The bottom Identity Authentication section should be filled out completely. In an ongoing effort to protect taxpayers from identity theft, the IRS, state tax agencies and the tax industry do ask for the taxpayers’ driver’s license number or a state-issued identification number. This information is optional for most state tax returns, but a few states do require it to complete the electronic filing process. It is also optional for a federal tax return. However, providing these identification numbers helps the IRS verify your identity which can prevent unnecessary delays in tax return processing. You also can review state revenue department websites for information.

Form I-9 Employment Eligibility Verification
Form I-9 is used for verifying the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must ensure proper completion of Form I-9 for each individual they hire for employment in the United States.

Form W-4 Employee’s Withholding Allowance Certificate
Complete this form so that your employer can withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes.

Form W-9 Request for Taxpayer Identification Number (TIN) and Certification
Use this form to provide your correct TIN to the person who is required to file an information return with the IRS to report, for example, income paid to you, real estate transactions, mortgage interest you paid, acquisition or abandonment of secured property, cancellation of debt, or contributions you made to an IRA.

Tax Organizer
The tax organizer is a questionnaire designed to help you gather information needed for your tax return. The one provided here is a very basic version for new clients. Existing clients will have a more personalized organizer sent to their portals or mailed to them each January.
